The overarching theme is that his underlying metrics are very strong. Fastball velocity (96.4 mph), chase rate (32.7%), barrel rate (4.0%), and hard-hit percentage (228.0%) are all elite following surgery and over a year layoff.

Cole wouldn't be the first pitcher to return to dominance after Tommy John surgery. Avid baseball fans may remember that Justin Verlander won the 2022 American League Cy Young Award after surgery sidelined him in 2021. Heck, Jacob deGrom won back-to-back NL Cy Young awards a few seasons after having the surgery.

Innings Ramp and Short-Term Limitations

He may be going six innings deep, but Cole is on a short leash as he works back to having a full starter's workload.

The Yankees are holding him to less than 90 pitches per outing, which has been doable in his first three starts. (He has thrown 72, 79, and 83 pitches thus far.) He could realistically ramp up to 100 pitches per outing (the standard for starters) by late June, early July.

There's no exact timetable, though, and the Yankees won't want to rush him back. Cole could even skip a start to keep him from backsliding or getting fatigued, which fantasy managers will have to be aware of.
