Activia is giving you back a rebate check for up to $36 on your Activia yogurt purchases made between Dec. 22, 2011 and March 15, 2012.

All you need to do is click here to print off a rebate form.

Follow the instructions and submit the form with your original cash register receipts with the purchase price circled. You will be reimbursed for the amount paid on the purchases, up to $36.

There is a limit of one submission per family, household or address. Your rebate request must be postmarked by March 31, 2012.
